What does 'taking responsibility' mean? Or what it should mean? When people say it, do they mean that they are the one to take the blame? Or is it just said by some people in position to look good and do nothing at the same time?

Taking responsibility means doing everything in your power to do own the work itself. If it fails, then the failure is under your ownership. If it succeeds then it is due as credit to your ownership.
